Why Does Pioglitazone Cause Anemia?

Fluid Retention: Thiazolidinediones (the class of drugs pioglitazone belongs to) commonly cause fluid retention and mild edema. Historically, this was thought to dilute the blood, but studies show it also suppresses the bone marrow slightly, making red blood cell production less efficient.
Bone Marrow Effects: Pioglitazone can have a direct, mild suppressive effect on the bone marrow where red blood cells are made.
Residual Effects: Research has indicated that these lower red blood cell counts may persist even after pioglitazone is discontinued
