Intra-vascular ultrasound(IVUS) is a critical high resolution imaging modality in cardiologynused to visualize the interior of the coronary arteries providing 360 degree cross sectional view of the vessels.

It surpasses traditional angiography by allowing physicians to measure vessel dimensions, characterize plaque, and guide stent placement, particularly in complex cases like left main diseases, bifurcations, and chronic total occlusions. Recent studies show that IVUS-guided PCI (Percutaneous Coronary Intervention) significantly reduces major adverse cardiovascular events (MACE), including mortality and stent thrombosis

Clinical Applications of IVUS
  • Lesion Assessment: Evaluates the severity of stenosis, especially in "intermediate" lesions (50-70 percentage blockages) where angiography is uncertain.
  • Plaque Characterization: Identifies the type of plaque (calcified, soft, lipid-rich) to determine the need for pretreatment with plaque modification tools like atherectomy.
  • Stent Optimization: Ensures proper stent expansion and apposition (adhesion to the vessel wall), reducing the risks of stent thrombosis or restenosis.
  • Complexity Management: Crucial for left main coronary artery disease, bifurcation lesions, and long, heavily calcified lesions.
  • Low-Contrast Procedures: Allows for accurate measurement and guiding of stents with minimal use of dye, which is safer for patients with renal failure

IVUS vs. Other Imaging
  • IVUS vs. Angiography: While angiography shows a 2D shadow of the lumen, IVUS shows the 3D vessel structure.
