Diabetic chorea is a rare neurological manifestation of uncontrolled hyperglycemia which is characterized by involuntary sudden and fast moments usually involving one side of the body. It responds well to management of hyperglycemia with adequate hydration and insulin therapy.

Key Aspects of Diabetic Chorea:
  • Symptoms: Involuntary, jerky, random movements (chorea) or large-amplitude, flinging movements (ballism), often affecting one side (hemichorea).
  • Cause: Associated with high blood sugar without significant ketone buildup (non-ketotic hyperglycemia), often in older patients with type 2 diabetes, though it can occur in type 1.
  • Diagnosis: Primarily diagnosed using brain MRI, which typically shows high-signal intensity (hyperintensity) on T1-weighted imaging in the basal ganglia (specifically the putamen).
  • Treatment & Prognosis: The primary treatment is correcting blood glucose levels. Symptoms usually improve within days or weeks of controlling hyperglycemia, often with an excellent prognosis.
  • Reversibility: While generally reversible, failure to recognize and treat it early can lead to persistent movement disorders or permanent damage
